CHR Home Visits

We are now offering a  Home Visit service to natives living in the Sac and Fox Community.  Examples of who may benefit from this service include (but not limited to) : people with a chronic health condition such as diabetes or hypertension and would like help monitoring their blood sugar or blood pressure, new mothers/first time parents who have questions about caring for a newborn, or elders who would like to have someone just simply check on them once a week to see how they are doing.  If you, or someone you  know, could benefit from having an in-home nurse visit please call Grace at the CHR office to schedule.  If you are calling to refer someone, please also let them know that I will be contacting them and that you referred them for our service.  If you have questions, please don’t hesitate to contact me.  I am looking forward to meeting you!  Please note:  This service is not meant to replace regular visits with your health care provider and in case of an emergency you should always call 911.

 

SDPI 2016

 

The Sac and Fox Nation of Missouri is launching the newly funded 2016 Special Diabetes Program for Indians. The SDPI  Diabetes Best Practice that our program will focus on is Diabetes-related Education. Our Required Key Measure is: the number and percent of individuals in our Target Group who received education on any diabetes topic, either in a group or individual setting.

 

General Transit Program

We offer General Public Transportation at a reasonable rate. Need a ride? Give us a call at (785) 742-7471 ext. 2502, 2503, or 2500. We operate on a first come first serve basis. We offer discounted rates to Senior and Veterans.
